Data Controller: This clarification text has been prepared by Vefa Özel Sağlık Hizmetleri ve Ticaret Limited Company (“VefaDent”) in the capacity of data controller within the scope of Article 10 of the Personal Data Protection Law No. 6698 and
communiqué on the Procedures and Principles to be Followed in the Fulfillment of the Obligation to Inform in order to be transparent and accountable as well as to fulfill the legal obligation. The disclosures referred to the website have been prepared in this disclosure text and the disclosures regarding the employee, employee candidate, service recipient, potential service recipient, visitor, shareholder/partner and related person are separately regulated according to VefaDent’s Disclosure Obligation Procedure, provided that they are compatible with VefaDent’s data processing inventory.

Service Recipient and Potential Service Recipient; Personal Data Purposes Credentials Contact Information, Health Information, Patient Process Information, Finance, Visual and Auditory Registrations Execution of emergency management processes, activities in line with the legislation appropriate execution, execution of financial and accounting affairs, communication execution of activities, execution/supervision of business activities,

Visitor; Personal Data Purposes Physical Space Security and Health Information Visitors’ silent camera recording, HES Code and fever measurement degree personal data, provided that it is connected, limited and measured for the purpose for which it is processed; is processed for the purposes of carrying out emergency management processes, ensuring physical space security , conducting audit / ethics activities and ensuring the security of data controller operations. d. Website visitor; Personal Data Purposes Identity Information, Contact Information and Customer Transaction Information The name, surname, telephone, e-mail and message personal data of Website Visitors are processed for the purposes of conducting communication activities, tracking requests/complaints provided that it is connected, limited and measured for the purpose for which it is processed
Method and Legal Grounds for Collecting Personal Data: Personal data related to the visitor; Article 5/II-a of the Law No. 6698 on the Protection of Personal Data and pursuant to Article 5/II-f, “it must be explicitly stipulated in the law and the fundamental rights and freedoms of the person concerned Provided that it does not harm, data processing is mandatory for the legitimate interests of the data controller “is a legal reason”, in an automated way, Personal data of service recipients and potential service recipients; Personal Data No. 6698 Pursuant to Art.5/I, Art.5/II-a, Art.5/II-ç, Art.5/II-e and Art.5/II-f of the Law on Protection of Personal Data, “obtaining explicit consent, It is processed by automatic and non-automatic methods based on the legal reasons that it is clearly stipulated in the laws, it is mandatory for the data controller to fulfill its legal obligation , data processing is mandatory for the establishment, use or protection of a right and data processing is mandatory for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the person concerned.
Silent camera recordings taken to ensure the security of the physical space are processed by automatic method, by camera shooting, based on the legal reason that “data processing is mandatory for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the data subject pursuant to Article 5/II-f of the Law No. 6698 on the Protection of Personal Data”.
To Whom and For What Purpose the Processed Personal Data Can Be Transferred: Your personal data in question can be transferred to real or private legal entities in order to provide information in order to receive services from real or private legal entities by VefaDent and to authorized public institutions and organizations in order to provide information limited to the purpose requested by authorized public institutions and organizations within the legal authority.
Rights of the Relevant Person Pursuant to Article 11 of KVKK: The rights of the relevant persons whose personal data are processed are regulated in Article 11 of Law No. 6698 . The application rights in Article 13 of the same Law can be used through the contact information given below with the methods specified in the Communiqué on the Procedures and Principles of Application to the Data Controller .
The applications to be made by the relevant person will be answered and finalized by VefaDent as soon as possible and within 30 days at the latest.
